2.Ethos IUL (Indexed Universal Life)

3.Ethos Final Expense
This is a policy that covers final expenses (funeral & burial expenses at the end of the term). This policy is low-cost and is typically designed for seniors or individuals who require minimal coverage.
Physical and Medical Conditions (Knockouts)
Liver disease or disorder
Each policy of Ethos Life Insurance is subject to a pre-existing liver disease. They are as follows:
Prime / Select (Term Life Type) : Applicant is not eligible to apply if they have had any liver disease other than Hepatitis A, B, or C in the last 5 years.
- In short: You cannot apply for Prime/Select if you have had any major liver disease in the last 5 years.
Spectrum (Term Life type) : You cannot apply if you have been diagnosed with any liver disease in the last 10 years.
- Eligibility for Spectrum policy is strict and sees a 10-year limit.
IUL (Indexed Universal Life): You cannot apply if you have had any type of chronic liver disease in the last 5 years.
- This is a policy with coverage and cash value growth, so medical history is looked at mainly.
Final Expense: You cannot apply if you have had liver-related diseases in the last 2 years.
- This is a policy with limited coverage, but there are still some eligibility restrictions.
Kidney disease or disorder
Each policy of Ethos Life Insurance may or may not apply depending on the pre-existing diseases related to the nose or urine. They are as follows:
Prime / Select (Term Life type): The applicant cannot apply if he has a history of kidney failure, polycystic kidney disease, chronic kidney disease, or dialysis, except for cesarean.
- In short: Prime/Select policy is not allowed if he has major and serious kidney diseases.
- Spectrum (Term Life type) : Applicant cannot apply for a Spectrum policy if he has been diagnosed with or treated for a kidney-related disease in the last 10 years.
- IUL (Indexed Universal Life): Applicant cannot apply for IUL policy if he has had any chronic kidney disease in the last 5 years.
- Final Expense: Applicant cannot apply for a final expense policy if he has been diagnosed with a kidney-related disease in the last 2 years.

Respiratory disease or disorder
Each policy of Ethos Life Insurance has a specific coverage for respiratory and respiratory-related diseases. They are as follows:
Prime / Select (Term Life type): Applicant is not eligible to apply if he/she has a past history of COPD, emphysema, chronic bronchitis, pulmonary fibrosis, cystic fibrosis, or central sleep apnea.
- Introductory rule: Only cases like treated obstructive sleep apnea, mild asthma, or a pulmonary embolism that occurred 5 years ago are exempted from the rule.

Autoimmune disease or disorder
Each policy of Ethos Life Insurance has an autoimmune disease option, which may or may not apply. They are as follows:
- Prime / Select (Term Life type): Applicant cannot apply for a Prime/Select policy if he/she has lupus/SLE or scleroderma.
- Spectrum (Term Life Type): Applicant cannot apply for a Spectrum policy if he/she has been diagnosed or treated for diseases like Crohnโs disease, Uulcerative colitis, orpancreatitis in the last 10 years.
- IUL (Indexed Universal Life): Applicant cannot apply for IUL policy if he/she has had any chronic autoimmune disease in the last 5 years.
- Final Expense : Applicant cannot apply for a final expense policy if he/she has been diagnosed with autoimmune diseases in the last 2 years.
Connective tissue disease or disorder
Each policy of Ethos Life Insurance can or cannot apply to connective tissue-related diseases. They are as follows:
- Spectrum (Term Life Type): Applicant cannot apply for a Spectrum policy if he/she has been diagnosed or treated for connective tissue disease in the last 10 years.
- IUL (Indexed Universal Life): If the applicant has had any chronic connective tissue disease in the last 5 years, then he/she cannot apply for an IUL policy.
- Prime / Select / Final Expense:
- Connective tissue diseases are generally not applicable for these policies.
- Or if these diseases have been present for the last 2 years, then he/she cannot apply.
Organ transplant
Each policy of Ethos Life Insurance has an organ transplant option, which varies according to the policy:
- Prime / Select / Final Expense: If the applicant has any organ transplant, then he/she cannot apply for these policies.
- Spectrum (Term Life type): If the applicant has undergone or received treatment for an organ transplant in the last 10 years, then he/she cannot apply for a Spectrum policy.
Criminal history
Ethos Life Insurance has rules regarding eligibility for each policy based on the applicant’s criminal history:
- Prime / Select / Final Expense: Applicant cannot apply if they have been convicted of a felony or misdemeanor, a continuing felony, or on probation or parole in the last 10 years.
- Spectrum (Term Life Type): Applicant cannot apply for a Spectrum policy if they have been convicted of a felony, on probation/parole, or have a continuing felony in the last 10 years.
- IUL (Indexed Universal Life): Applicant cannot apply for an IUL policy if he/she has a felony or misdemeanor in the last 2 years or has a continuing criminal offense.
Drug/Alcohol use
Ethos Life Insurance has rules regarding the eligibility of each policy based on the applicant’s alcohol and drug use:
- Prime / Spectrum / Final Expense: The applicant cannot apply for these policies if he/she has alcohol or drug abuse in the last 10 years.
- IUL (Indexed Universal Life): Applicant cannot apply for an IUL policy if he/she has alcohol/drug abuse in the last 5 years.
Disability
Ethos Life Insurance has rules regarding eligibility for each policy based on the applicant’s disability status:
- Prime / Select (Term Life type): If the applicant has been unable to work for more than 30 days in the last 12 months, then the Prime/Select policy cannot be applied for.
- Spectrum (Term Life type): If the applicant is currently permanently unable to work, receiving disability benefits, or hospital confined, then the Spectrum policy cannot be applied for.
- IUL (Indexed Universal Life): If the applicant has a chronic illness or permanent injury/disability, then the IUL policy cannot be applied for.
- Final Expense: If the applicant has been hospital confined 2 or more times in the last 6 months, then the Final Expense policy cannot be applied for.
Driving
Ethos Life Insurance has rules regarding eligibility based on the applicantโs driving history for each policy:
- Prime / Select / Final Expense: Applicant cannot apply if they have committed DUI/DWI, hit-and-run, or reckless driving in the last 5 years. Or if they have had a driver license suspended/revoked in the last 1 year.
- Spectrum / IUL: Driving history is generally not applicable for these policies; eligibility does not affect eligibility.
